This Funders Group combined efforts to provide an Outcome Based Evaluation (OBE) process, so agencies can evaluate their ability to reach their desired outcomes.
The Pierce County Funders Group and the Outcomes Implementation Committee presented at the 2009 Outcomes Summit the full spectrum of the Outcomes Based Evaluation process as it pretains to funding decisions.
As referenced at the Summit, 2007 Performance Measure Profiles are listed below that summarize resource, output and funding data by service strategy. Please note that the data reflects only programs that contracted with the City of Lakewood, City of Tacoma, and/or Pierce County Community Services. Annual Report and Semi-Annual Data Sheets:
Agencies funded by the Pierce County Funders Group are required to submit a report annually on the success of their program's ability to reach their desired outcomes. Most agencies report on a regular calendar year schedule, January through December, submitting Semi-Annual Data sheets (representing the first six months of collected data) due July 31, and an annual report (representing 12 months of collected data) due January 31 of the following year. A few agencies find it necessary to request an exception to use the alternate reporting schedule, July through June, where the first six months of data is submitted on the Semi-Annual Data sheets January 31. Their annual report is due July 31. Only the annual report requires additional financial information on its cover page.
The tools below are used for this yearly submission.
- The Semi-Annual Data Sheets form is used once a year alternating with the common report form. This form represents the first 6 month's data collection results. This form is revised accordingly to due dates either July 31 for regular annual reporting or January 31 for alternate schedules .

- The Common Report Form is revised according to its due date and is used for annual reports due either on, January 31 or on the alternate schedule, July 31. The data sheets on this form should represent 12 months of data collection. Please complete the requested financial information on the cover sheet.

Agencies who are new to the OBE process have a transitional period to prepare them for an annual report. Below is a link that provides the schedule that establishes quarterly benchmarks for tracking progress. Agency staff should attend OBE process training and annual report training. Technical asistance is also available from their funding contact. Two different fiscal year schedules are identified.
Also listed, is the onsite monitoring tool for the transitioning process that scores an agency's progress. This score is used for their future application process.

Agencies funded by the Pierce County Funders Group are required to submit a Data Collection Worksheet (DCW) that displays their outcome choices, data collection system, and their criteria for measuring success. The DCW is used as a guideline to perform an onsite monitoring visit of those components. Standardized tools are used by the funders to rate and score their findings, and the results are used in future funding decisions. Below is the DCW form and the monitoring tools used by the Funders:
