To submit an address and/or phone number change, please complete the appropriate forms and return them to the Human Resources Department or other organization as detailed below.

Regular employees making dependent and/or name changes, please refer to the Dependent Change page also as there are additional forms required.

Extra Hire employees needing to make a name change in addition to an address or phone number change, are required to submit a copy of their updated Social Security Card, along with the appropriate forms indicated below, in order to complete the name change process.


Employee Information Form (EIF) - The completed and signed Employee Information Form is required to make changes to your personnel record. Return the form to the Human Resources Department.
Regence Blue Shield - If you are a participant in one of the Regence health insurance plans; complete, sign, date and return the Regence Enrollment and Change Form to the Human Resources Department. Regence does not accept changes directly from members of our group; forms must be signed by a Pierce County Human Resources representative.
Options/Group Health - If you are a participant in the Options/Group Health insurance plan and use the inside Group Health network, you will change your address directly with the office you are receiving services/care. If you are using the outside Group Health network; complete, sign, date and return the Group Health Enrollment Form to the Human Resources Department. Changes may also be made on-line at the Group Health web site.
Washington Dental Service - If you are a Washington Dental Service member; complete, sign, date and return the Washington Dental Service Enrollment Form.
Willamette Dental of Washington - If you are a Willamette Dental member, contact them directly at 1-800-360-1909 to update your records.

ICMA Retirement Corporation - If you participate in the ICMA Deferred compensation program call 1-800-669-7400 or use Account Access to make an address change. Need other ICMA change forms?

ING (AETNA Financial Services) - If you participate in the ING deferred compensation program; complete, sign, date and return to the Human Resources Department an ING Change Form.
Personal Choice Account - If you participate in the Personal Choice Account, flexible spending account; complete, sign, date and return to the Human Resources Department a Personal Choice Account Change Form.

United States Savings Bonds - If you purchase savings bonds through payroll deduction, request a change form from Payroll or the Human Resources Department. Complete, sign, date and return the form to the Payroll Division of the Budget and Finance Department.
